Broken polls / surveys was my experience on the forums, but it was a year or so ago. From what I remember everything would setup fine, but nothing got recorded, could be as simple as incorrect write premissions somewhere within phpBB, if someone wants to investigate.

I've un-announced this topic since it's was originally just an early notice of the annual meeting. Discussions of the poll results can continue, or not, whatever. It'll just be a normal topic from now on though. perhaps when the poll results are done, a new post announcing them can be made
